Does this require me to alter my exhaust?

If you have a 2.5" cat-back exhaust that closely follows the stock routing, then it should fit. MM says that most of the time when exhausts don't clear its because the tailpipes come down after the rear end too soon. If that's the case, you'll have to add a straight section of tubing just after the muffler to move the "hump" of the tail pipes back.

I have a stock catback with flows welded in.

You'll clear it with stock catback or with flowmaster's catback, so stock with flows welded in shouldnt be a problem...
